I work in a hospital setting... trust me I know...

Fine tuning is definitely needed, but sometimes its like picking a
needle out of a haystick and the needle is the same color as the hay...

Clear as mud right?

Try this in  a hospital setting. A simple search word like breast would
be very common in this setting but would also kick off a multitude of
porn sites.
We are rolling out a new Barracuda web filter and it amazes me looking
at the traffic. I can tell this is going to take some work to fine tune
it though.

And for the most part it requires a human to figure out what it is. Is
it art or is it porn for example.    To quote Justice Potter Stewart:  '

  Web filtering suffers from the same problems as signature-based
anti-virus, but orders of magnitude worse.  There's too much stuff to
ever hope to keep track of it all; there's too much legitimate stuff
that one can't predict; it's not like software where one can often limit
the field to a handful of things needed.
