We submitted this to our web development staff and they wrote a quick webpage for this purpose. I will not say it is totally anonymous, however if you wanted it to be you could use one of our break room computers. If you are at your desk, I could probably trace it back to your computer, but that is as far as it goes. So far no one has even asked for that, and I think my assistant and I are about the only ones who know we could do that and how.
