I think I am going to try surveymonkey.com. I am told that it just logs IP
addresses so it would just show our public IP so that would be anonymous
enough.

We submitted this to our web development staff and they wrote a quick
webpage for this purpose. I will not say it is totally anonymous,
however if you wanted it to be you could use one of our break room
computers. If you are at your desk, I could probably trace it back to
your computer, but that is as far as it goes. So far no one has even
asked for that, and I think my assistant and I are about the only ones
who know we could do that and how.

suggestions or list critisms of the company yet have it be anonomous? If
