Have you tried Adobe Reader? I'm pretty sure you can use it to fill out
forms and add sticky notes to a PDF.

If that won't do it, the only other way I know of is the full Adobe
Acrobat product.

2 alternatives to Adobe Acrobat to consider are:

Nitro PDF Professional: http://www.nitropdf.com/products.htm
Nuance PDF Converter 5: http://www.nuance.com/pdfconverter/professional/

BTW Any of the for-pay Adobe Acrobat products will do what you want.
Adobe Standard is the cheapest @ $300-ish US, Pro and Pro Extended are
much much more expensive - $500-ish and $700-ish respectively.
