It really depends on what you are trying to monitor. Just uptime? Service status? Synthetic transactions? Windows or a mixed environment?
We have an older version of BigBrother, which is now Xymon (http://xymon.com/ ). It works very well for what it does. I've also used NetIQ's AppManager. It works, but I don't love it. Not great in a mixed UNIX/Windows environment. I'm also working with SCOM 2007 right now and it seems very powerful. I've also used What's Up Gold in smaller environments with good success.
