We use SpamTitan, which is a FreeBSD based product using a bunch of technologies including SpamAssasin, Clam AV and Kaspersky AV. It has a lot of features and is reasonably priced. Available as a VMware Appliance (which is how we are using it), a physical appliance or it can be installed from an ISO onto your own equipment. It works very well for us. I agree that it would be good to use an off site filter and save bandwidth, but the cost of those services is always too much for us, being a non-profit on a tight budget.
