Back in the day, say 2003-04ish  I was running Smartfilter from a company 
called N2H2.... that did this in a very similar fashion and I loved it.  This 
company was purchased by Secure Computing who then was purchased McAfee...not 
sure if that feature is still in the product or not.  It was nice because a 
user might say "Hey I need access to domainx.com, but really it was redirected 
the content from domainy.com that was being filtered.  The block page had a 
link to submit a review request which consisted of a form for the user to input 
their reason why.  The system then delivered me an email containing the AD 
account, the IP of the wks, the full URL they were trying to access and 
filtering and their reason.  Reviewed them once a week and took appropriate 
action.

All,

Anyone know of a web filter that can require attestation that the user
believes the site to be work-related and safe for each new domain
visited, also announces that all web visits are logged and monitored?

I envision that it would keep state for some amount of time (several
days/weeks/whatever) so that it wouldn't require the attestation each
visit.
