Do you use the exact same URL for LAN/WAN? Check the site on both LAN and WAN and see what zone IE puts it in. By default Automatic Logon is only enabled for the Intranet Zone.

Integrated Windows authentication doesn't work if you're not logged on to the domain, so in the external scenario it reverts to basic authentication. You can go in to the IIS Manager snapin and check the directory security settings on the site in question.
