Usually this formula works great:

Focal length/F number = Diameter of the Aperture/2 = Radius of the Aperture 

So first of set your film back and focal distance and then lets say you have a 
50mm lens with f/4

So 50mm/4 = 12.5mm that is the actual diameter of the aperture 

12.5/2 = 6.25mm is the radius you want to use.

Also pgBokeh has some cool features and fairly accurate if you have the data.
