We wish to revert to e-conjsult and leave patches

This is not limited for 2 months but as agreed at yesterday's meeting until 
such a point that patches or an alternative procured, is fit for purpose and 
patient safe

Please, we must make it clear.  We will not risk a system which threatens 
patient safety

The optimism provided in your email are not satisfactory for the practices in 
K&S S

The option to limit the number of patient encounters cannot be predicated on 
the system we decide to use and must be removed and corrected.  In terms, 
again, of patient safety is not acceptable NHS non-clinicians imagine the GP 
workforce has infinite capacity

Dear Dr De Kare-Silver and Lade

We understand that your practice has requested to extend the eConsult contract 
for a maximum of two months.  This is to allow the practice time to address any 
challenges that you are experiencing with the transition to PATCHs before 
making a permanent switch to this platform.

To select the appropriate approach for your practice, we are proposing the 
following 2 options:

OPTION 1 - To remain with PATCHs with the ability to limit the number of 
requests received using the flexible demand management limiter. This will help 
practices plan clinical and admin capacity appropriately. Additional tailored 
1:1 support will also be provided.
Should option 1 be selected no further action is required.

OPTION 2 - To extend eConsult for a maximum of 2 months, either setting PATCHs 
access to zero, which means no patient access through PATCHs, or use a hybrid 
model, which would mean eConsult is your main platform, while working with a 
selected cohort of patients in PATCHs.
Should option 2 be selected you need to follow these steps:


*         To help the practice resolve this complex situation, and manage the 
expectations of practice staff and patients, we would like to discuss options 
for the practice with your lead GP partner on a brief Microsoft Teams call 
today.


*         Set up a feedback session for us to better understand the challenges 
the practice has encountered, covering the discussion points outlined below.



*         Confirm the process of extending eConsult, and confirm the new Go 
live date for PATCHs ensuring patient communications are in place.

Additional Discussion Points Outline for Practices choosing Option 2

In order for the NWL Digital First team and PATCHs to resolve challenges, we 
would like to gather the issues that practices are experiencing.

We will use the below points to support discussions and help us to better 
understand the specific challenges experienced with Patchs tool. This will help 
us provide feedback to the supplier as well as tailor our support to practices.



1.       What are the specific challenges that you are experiencing when 
processing Patchs Online Consultation requests?



2.       Do any of the challenges you are experiencing relate to any of the 
following? (Please expand on points where helpful)


(a)    Free text from patients compared to the detailed questionnaire and what 
type of addition clarification questions/additional information need to be 
provided?


(b)    The availability of pre-formatted Clinical templates?



(c)     The categorisation of the clinical acuity of the Patients request and 
triage within the practice?



(d)    Challenges with the Interoperability with EMIS?



(e)    Challenges with Patient registration?





3.       Has the practice utilised the Translation and Artificial Intelligence 
options?





4.       Are there any specific challenges with the demand management limiter?





5.       When would be your practice target for a permanent Go live date with 
PATCHs that is achievable, within the extension window of two months?
