I posted a question re opcode caches and segfaults the other day, thought I should follow up now that we found the cause :)
The issue seems to have been the combination of Apache modGeoIP commands in .htaccess AND an opcode cache. We didn't see the segfaults before installing the opcode cache, and GeoIP was in .htaccess was there for a month or two before that. Moving the GeoIP code from a per-directory .htaccess to the site's virtualhost entry fixed the segfaults. Wierd, but delighted to have made them go away ...
