Harvey - if your site is built on a framework that uses page caching
for improved performance (as Drupal does optionally) you may need to
shorten the cache lifetime. In the early days of using Drupal this
caught me out as modified pages, especially the theme graphics, just
wouldn't update, Control F5 had no effect.

Sometimes this can also be a case of the local machine caching older versions
of the site - you may be able to verify this by performing a Control-F5 within
Internet explorer.
I notice that your expiry tag is also very much in the past - I'm not 100%
sure, but that could be causing some confusion to proxies / web browsers.
