What year? 2003 or 2007+? If the latter, it does not support background images since it uses Word's HTML engine to render (03 uses IE with and option to use Word)
There are a few work arounds out there. Here's one but I haven't tried it yet: http://www.campaignmonitor.com/blog/post/1777/body-background-images-outlook/. Simpler (And more robust) solution has always been to give the container a background color to fall back on. Gmail has issues with background images too.
