Yep, I suspect this was caused by a recent PHP upgrade.

/usr/local/lib/php/extensions/no-debug-non-zts-20060613/ffmpeg.so does exist.

The PHP extension dir in php.ini is /usr/local/lib/php/extensions/no-debug-non-zts-20060613

I can't however find libavformat.so.51 which I'm assuming is a requirement for ffmpeg.so to load.

Was I supposed to do something special when upgrading PHP to ensure ffmpeg continued working?

If PHP has been upgraded it might have installed the new ffmpeg extension in a 
new location so you'd just need to update the path in your php.ini
