looks like php extension was built against older version of libavformat
try to do
ln -s /usr/lib/libavformat.so.51 /usr/lib/libavformat.so.52.64.2
not sure if it solve the issue

/usr/local/lib/php/extensions/no-debug-non-zts-20060613/ffmpeg.so
From the research I have done, it seems to be an issue with
ffmpeg-php rather than ffmpeg itself.
