Hi Daniel...welcome to the OBC.
Sorry to hear you are cursed with wide feet. Size D width is the standard width for
men, then comes E and then EE or F (same thing). I do not think that size is
particularly uncommon. I checked some bike shoe makers and could not find any
references to shoe widths. All I found on Shimano's web site is:

You might have to special order them. If you can, get shoes with as much leather
around the toe as you can find - leather will stretch. A shoe store usually has
shoe trees they can use to stretch the leather.
Another option is to get a pair that are too long, but wide enough. The trade off
is there will be a gap between where your toes end and where the shoes end. You
will also have to adjust the location of your cleats. You have to make sure that
your foot does not move around inside the shoe, otherwise, you will waste energy on
each revolution. There are guys in the club who know far more about that sort of
thing than I, such as Bob Simpson.
