If the threads are worn in the location where the cones were on the axLE, you've stripped the threads there.  This is usually due to over tightening.  You must replace the axle because the cones won't be adjustable and in fact will move about as if they had a mind of their own.  This is a regular maintenance item and I'd recommend that more people look there to save grief in the future.  Mind you, just the removal and retightening of the cones will tend to damage the threads if you aren't very careful to not over tighten them.

Thinking of North Carolina, I removed the axel from my rear wheel
today to re-grease the bearings. I found that the threads on the axel
where the cassette covers it appeared worn down more than the rest of
the axel. I do not recall seeing this condition before. Is this
normal, or should I replace the axel? If this is not normal, what
caused it? Did I tighten the cones too much?
