Troubleshooting:

1) Are you sure that the wheel is true, both radial and lateral? (radial
= up and down, lateral = side to side)
2) Is the dish ok? (is the rim centred about the hub)
3) Does the tire have wear spots?
4) Is there too much (or too little) contact between the roller and the
tire?

When I'm on the trainer with one of my almost-true rear wheels, I really
notice the vibration. On the road with this wheel, nothing! In the wheel
building stand, the radial true is out by a few mm (dips and peaks as
the wheel goes around).

To answer your first question, yes there are balancing techniques
(Torelli's (really Ambrosio's)
(http://www.torelli.com/parts/wheels.html) top road rims have a little
weight around the valve stem to compensate for the slight loss of metal
in the hole). But really, the valve makes up for most of the weight lost
in the rim hole.

A bike wheel is "balanced" by the addition and removal of spoke tension;
a car wheel is balanced by the addition and removal of weights.
