Service error when adding multiple invoice items

Apologies in advance that I cannot access a test system to recreate this and
get the detailed error message...
If you create an invoice and then try to add additional items, the first
addition succeeds with no problems. When you add a subsequent item you get an
error telling you that the service cannot "set-nonpk fields" (again apologies
for the slightly vague error report).
If you go straight back down to the item add form at the bottom of the screen
and hit "Add" a second time, it succeeds - odd.
