Yes, it is when I read it. OGC and PI must be exclusive.

If you want private PI, then you're going to have to do something to
separate the from the OGC. The above is not my first choice, but yes, the
license demands that they each be clearly marked in some way.

Yes, with the caveat that you can't allow the identification to overlap. By
this I mean the title cannot be construed to be in both PI and OGC.
This is fine:
"All items in this document are Open Gaming Content except for instances of
'Dougworld', which is Product Identity".
But this has a potential problem:
"All plain text is OGC. 'Dougworld' is PI." and then have 'Dougworld'
appear in plain text, because the two statements cannot both be true. If
'Dougworld' only appeared in Bold or in Red, then that would not be plain
text, and everything would be fine.
