Thanks for the answers.  I have read both the OGL and d20 Sys Lic, at least
twice each (probably more) and did not get an answer, so I turned to another
resource, this list.

No you cannot refer to page numbers. Yes you must
include the SRD in your Section 15 if you use content
from the SRD.
