This is the new ODOT e-permit system: 
https://www.transportation.ohio.gov/wps/portal/gov/odot/working/permits/e-permitting-system-support

I have not used it yet.

The old proecess required filling out a MR-505 form (fairly simple) and 
submitting drawings.   There did not seem to be any consistent standards 
published for what needed to be on the drawings.  The local (District 2) permit 
person required things that I’m not sure are required everywhere.
