Another thing that I noticed with the recent Goethe mess is that the same merge looks like it go executed twice in a row. http://openlibrary.org/recentchanges/2012/10/06/merge-authors.json I'm not sure if it was a user error or a software bug or ...?, but might be worth looking at if your looking at the merge code.
