Tom, when I looked at this at one point I did find a few instances where substituting the $q for the string following the comma in the $a did not produce the desired effect. These may have been cataloger errors, since the $q is rather complex and I don't know if it is ever described as being a pure substitute for the forenames in $a. However, if it is used only for searching, not display, there probably is no harm from these oddities.
That said, while considering this we probably should consider storing the full library authoritative name in the OL record, which would then make it easier to link between names in OL and names in library data (e.g. to connect to VIAF).
