We have had a few reported cases where a patron placed a hold and then the hold request did not show up in the patron's account information in the OPAC. Other holds placed by the same patron did show up. But we are a very, very new Evergreen implementation, and many of the odd things we see turn out to be problems in our own set-up or in our understanding of how things are supposed to work, not actual problems in the software. We haven't had a chance to track this one down to see how to reproduce it yet. We are currently using v1.4.0.4.
