I agree with the expire date and would like if nothing else to have first name option. Would be nice for the parent with 5 kids and receipts for all.

Are there any strong opinions about adding more fields or removing some of these? I figure it's better to knock this out all at once rather than poking at it repeatedly with tiny changes.
