I can replicate this on 2.6.29, but not on newer kernels. I remember some bug fixes in the scsi or block layer went in for handling requeues and unplugging (originally the problem was seen with fibre channel and iscsi uses the same helpers as FC). I am looking through the kernel.org kernel git logs now, but am having trouble finding the fixes.
If you could give 2.6.32 a quick try and verify it works for you it might help.
