I do have some, yes, but it looks like they were only a second-hand symptom 
of the problem.  I've attached one even though I think I've solved where 
these were coming from. Anything in parentheses is where I attempted to cut 
out any sensitive data.
 
At first I was lost as to how certain pages were being called without their 
prerequisite variables (by various logged-in users at various locations).  
But this is what I think was going on here...  On most of my front-end 
pages, I include some code that checks via AJAX if the user is still logged 
in.  If not, there's a popup that allows the user to attempt to log back in 
without having to leave their page.  The login popup has a form in it that 
looks like this...

<form action="" onsubmit="dynamic_login.attempt(); return false;">

The assumption was that "return false" would always prevent the form from 
submitting.  The only reason I was using the form at all was so the login 
attempt could be triggered by the user hitting Enter within the password 
field as well as clicking the submit button.  However, I've discovered that 
whatever my overarching problem is, it's causing AJAX to occasionally error 
out.  In this dynamic login code, I did not have a try/catch around the 
AJAX call.  So it looks like the "dynamic_login.attempt()" would 
occasionally fail with that AJAX error, "return false" would not get run, 
and the form would submit, basically just refreshing the page but with any 
important url variables stripped and the form's password field added.
 
So this solves the mystery of these runtime errors I'm seeing, but I still 
have the big problem of what is causing our clients to frequently have 
failed connections.  According to our server admin, the performance and 
network logs don't indicate any problems at all and nowhere near any 
thresholds, so I'm thinking the problem has to be somewhere between Apache, 
Tomcat, openbd, and MySQL.
