Hmm, in my testing I did not get any of these problems, in fact the "approval" was one of the easier steps for me (only taking 69 minutes). Are you sure that you were not running multiple processes at the time ? This may explain the MySQL deadlocks. I noticed (by looking at the top processes (via "top") that the "ca" process was still working even though it seemed to have failed.
