1.3.6.1.4.1.311.20.2.2 is MS SmartCardLogon

Your keyUsage extensions look good.

Did you set "security profile"  in Tools->Options, tab Secity, button
Settings ?
Do you have CA cert in CAPI repository?
Problem could be, Outlook doesn't trust your certificate, because it
can't verify it.
