Michael,
I think the problem is that in your Guide (page 16) you wrote --with-hierarchy-level= *ra* at the installation of the CA. And at installation of the RA (page 17), you wrote --with-hierarchy-level= *ca* . You changed the parameters and it causes confusion for people who are starting with OpenCA! You should correct it as soon as possible!!!
This bug is known for a long time and not patchable :(
The result of this problem is that we start writing a complete new documentation for the next release where the source are manageable by CVS. The old documentation (until 0.9.1) was never managed by a versioning program and so we mixed the different version. The confusion will stop if we release 0.9.2.0.
