first I'm not a Professor. So please never use this title together with
my name. Second there is a user mailing list for exactly such normal
questions. Please see the OpenCA webpage or
http://sf.et/projects/openca/. Third the problem is simple.

If you look into the sourcecode of OpenSSL then you will see that this
error means DB_ERROR_INDEX_CLASH. This means that there is something
wrong with the file index.txt in OpenCA_DIR/var/crypto/. Usually there
is an additonal empty line attached at the end of the file or you try to
issue a certificate with a subject which is already part of the index.txt.
