Sounds like it could be a problem in your Openssl index file: /usr/local/OpenCA/var/crypto/index.txt.
We've experienced similar problems in the past which have been attributed to a corrupt index: possibly a duplicate entry. Latest versions of OpenCA have a command to rebuild this index: rebuildOpenSSLindexDB, although I'd make a backup of the index beforehand.
