Perhaps the index.txt in <Your-OpenCA-Dir>/var/crypto/index.txt is
garbled?   (var/crypto/index.txt is IMHO the default location of this
file; it is configurable with the sslindex-parameter in etc/servers/*).

In the node interface you can find a link named "Rebuild OpenSSL's
database and next serialnumber" under "Administration->Backup and recovery".

This is supposed to rebuild the index.txt that gives you the errors
"error creating name index:".

Perhaps this command can help. I guess you should backup var/crypto
before trying this... .
