I found the source of my bug.

In the registry: under
HKEY_LOCAL_MACHINE/Software/Microsoft/Cryptography/Calais/Readers,
I had two card readers listed:
Gemplus GCR400 0
and
Gemplus GemPC 400 0
which of course are the same thing.
I removed the older name (GCR400) from the registry and everything works ok
now again.

This occurred after I updated by GemPlus GemPC400 PCMCIA Smart Card Reader
driver from version 2.1.4.0 to version 2.1.6.0.

My OS is Windows XP.

Could this be related?

Even though my smart card reader is connected through PCMCIA, do COM1 and
COM2 still have to exist?

I am getting error 8010002e
com.ibm.opencard.terminal.pcsc10.OCFPCSC1.SCardGetStatusChange.

Does anyone know what this means and how to fix it?
