Rafiu I was having problems doing anything involving either LVMs or RAID of any kind when drives were hosted on either [a pair of] Adaptec 1210SA or [a single] AMCC/3ware 9500 controller. The level of the RAID made no difference; I was still seeing the same errors.
Underlying hardware was an Asus board with Pentium P4 3ghz cpu, 2gb ram. I'm not sure how much more diagnostic or contextual information you might need.
