John,
You are absolutly correct. This is a very big problem with Medical
Billing. This will be the primary focus of the FreeB system 2.0 mark (we
are not yet at 1.0).

The ultimate goal of the the FreeB Billing System, is to corrdinate
with a Free and Open Source Practice Mangement System, to provide
billing AND billing tracking. Most of the billing tracking will be
implemented within the Practice Management System, but as Tim Cook
implied in his last posting of the subject, unless there is good
reporting in the Billing Module, it will be impossible to make the long
term goal of billing tracking possible.

My hope is to make the billing tracking systems of the Free Medical
Software Community so excellent, that it becomes a competitive advantage
for free software! The ROI of an accurate billing system is the easiest
thing to sell doctors!
