The  best thought-of general practice appointments system in the UK runs on 
MySQL, the Windows version of  which is embedded in it.  (FrontDesk)

If it is runing against clincial software then it will be using the database 
from that for demographics etc.

Given the licencing of mySQL, that company must be paying a fee.
