try to switch off your firewall and try again
and maybe do the following
modify src/META-INF/mysql_persistance.xml
update to the latest revision
svn up
ant clean.all
ant -Ddb=mysql
cd red5\dist
red5-debug.bat

to work with mysql you need to:
set the correct host/port/db/user/password in mysql_persistance.xml
copy mysql_persistance.xml to persistance.xml
do the following:
inside dist/red5
run "start cmd" (new cmd window will start)
inside this window (black background)
run "red5-debug" (not red5.debug)
copy the text from cmd window and send it to me

what are you running?
have you built OM from the sources or download binaries?
have you modified the persistence.xml file?
which version of java do you have (java -version)
can you run
red5-debug.bat and send me console output?
