Which SVN revision are you running? I ran into a similar problem quite some time ago, but the problem turned out to be when you added language support to chan_capi, and the struct wasn't being populated with the language code from capi.conf properly. Without the language code in the struct, file.c wasn't searching directories properly, and reported that no suitable MoH formats were found.
