To use a LACP trunk, the switch needs to support it, and it needs the four ports must be configured into a single port-channel. Do you use auto-negotiation and is it set exactly the same on the switch and on the host? The reason why I ask these questions is because 3 failed ports on a switch is just too unlikely.
