Have you used zfs commands to detach/replace the disc before physically 
replacing it? (I am not zfs guru, and not sure what the commands should be - 
zpool detach? zpool replace?)

If not, maybe replacing back a faulty disk is the option (it contains a part of 
your pool, and the new one is not), and then replace it again in the proper way 
- with special commands before and after replacing.
