First thing: use digest(1) to make sure the image got downloaded okay. The md5 checksums for the 2009.06 images are posted on the downloads page. If the checksum is good, I think the maintenance password is either "opensolaris" or "jack". If one of those passwords works, you'll get a single-user shell prompt, from which you can look at system log files, run "svcs -x", etc. mike
