The above standard is also known as X.680, and can be purchased online
through the ITU's online bookshop at http://www.itu.int

The fee is only mildy extortionate, and the whole process takes about five
minutes if you have a credit card.

The best part is that the standards are delivered in either Word or
Postscript format, so you can print them out as many times as you like and
put the holes where you want!
