Does the passtos option need to be set BOTH on the server and client?
passtos is a "forwarding" option, that is with --passtos set then the
type of service of all traffic that comes in on the OpenVPN encrypted
channel is copied over to the forwarded traffic leaving the outgoing
interface (usually the ethernet interface). Having said that, it is
hardly ever required to set it on a client, unless that client is
forwarding traffic for a client-side lan
If so, can I "push" the option to the client?
yes you can

as long as the OS supports PASSTOS then the client will support it ; I
just checked an old WindowsXP VM with OpenVPN 2.3.18 and --passtos was
already supported then - so it is safe to assume all modern versions of
Windows support it. I expect the same for MacOS. Then again, I doubt
that you would ever need to set it on the clients.
