Hmmm, looks fine to me.
Can you try using a network sniffer or a Firefox extension like
Live HTTP headers to figure out which header is actually sent
to you? I can see no reason in the code why the CGI version should
behave differently - if $attachment is 0, it should not even execute
different code for CGI or mod_perl ...
