The menu is loaded after login, so on the first page it is ok if you just see "Clear Login" and the Dropdown to select the authentication stage. Can you please answer following questions:

* What do you see after sending the login form?
* Check if you have /var/openxpki/session and if its writable by apache
* Check the apache logs for webui.fcgi?page=bootstrap
* Attach contents of /var/openxpki/webui.log

My best guess is that you are missing the session directory so you dont get a session and so you dont get a menu send by the server.
