Team, Several people have told me that they got an error when they tried to use the form. The problem is that each form has the same title, so the wiki considers them identical and generates an error. I updated the title with "fields," in angle brackets, for the name of the project and the milestones affected. This should produce unique titles, even if the same project submits multiple requests.
