Ok, so you are creating a new version of the "business document" in the action (I assume it is event_model.xpl, if this is from the page flow you sent earlier), that new document is the output of the action, and you update /instance/document with this new document using XUpdate in the page flow. That looks fine to me.
Can you try to put a debug on the "data" output of the action, and on the "instance" input of config/epilogue.xpl? The later will show you the instance that should have been updated. If you don't get any useful information from this, would it be possible to attach to your response your page flow and all the files used in that page (XForms model, action, view, etc)?
