As discussed off-line, I tried to fix this bug and I think I managed to, even though I was getting a slightly different stack trace than you were getting :-) The new WAR can be downloaded from here (until we have completed the migration of our nightly builds):

You should be able to just update orbeon.jar. Please backup your old orbeon.jar before, of course. Let us know how this goes.

We have an important demo to our group presidents coming up on Monday and need to show the attach document functionality. However, this means a severe restriction on how large the xml instance can be (especially in IE which seems to have a much smaller limit than Firefox). If there is anything you can do on this or any other workarounds you can think of we would really appreciate it.
